APPLICATION FORM
A. RIGHTS OF THE DATA SUBJECT
Under Article 11 of the Law No. 6698 on the Protection of Personal Data, everyone has the right to apply to our Company, which acts as the data controller, for the following matters:
a) Learn whether personal data is being processed,
b) Request information if personal data has been processed,
c) Learn the purpose of processing personal data and whether they are used in accordance with their purpose,
ç) Learn about third parties to whom personal data is transferred, whether in Turkey or abroad,
d) Request the correction of personal data if it is processed incompletely or inaccurately,
e) Request the deletion or destruction of personal data in accordance with the conditions specified in Article 7 of the Law No. 6698,
f) Request that the actions taken under (d) and (e) be notified to third parties to whom the personal data has been transferred,
g) Object to the occurrence of a result against the individual by solely analyzing personal data through automated systems,
ğ) Request compensation for the damage in case of suffering any damage due to the unlawful processing of personal data.
B. APPLICATION PROCEDURE AND PRINCIPLES FOR DATA SUBJECTS TO APPLY TO THE DATA CONTROLLER
Pursuant to the 1st paragraph of Article 13 of the Law No. 6698 and the Communiqué on the Procedures and Principles for the Data Controller to Apply, published in the Official Gazette dated 10.03.2018 and numbered 30356, applications related to these rights should be made to our Company via one of the following methods, or other methods determined by the Personal Data Protection Authority ("Authority"):
The data subject may apply to the data controller as follows:
• In writing, • With a secure electronic signature, • Via Registered Electronic Mail (KEP) address, • With a mobile signature, • By sending it from the e-mail address that the data subject has previously notified to the data controller and is registered in the data controller's system.
The application must include the following mandatory information:
• Name, surname, and signature if the application is in writing, • Turkish National ID number for Turkish citizens, nationality, passport number, or ID number for foreigners, • Address for notification purposes or business address, • E-mail address, phone, and fax number (if available), • Subject of the request.
If available, related information and documents should be added to the application.
In written applications, the date on which the document is delivered to the data controller or their representative will be taken as the application date.
In applications made by other methods, the date on which the application reaches the data controller will be considered as the application date.
The application will be finalized in the shortest time possible and within a maximum of thirty days free of charge. However, if the process requires an additional cost, a fee may be charged according to the rate specified by the Authority.
Applications must be made by the person concerned. An application on behalf of someone else can only be made if it includes a request for information under KVKK and is accompanied by a power of attorney. If there is any suspicion about the identity of the applicant, the Company may request verification information from the concerned person.
